The Pre-Registration Process

We continuously strive to assure that our services are delivered in an efficient, caring manner. You can help us expedite your registration and thereby shorten the time you need to be here on the day of your procedure by following these guidelines:

·        You must come in at least one week prior to the date of your procedure to pre-register

·        Please bring a picture ID and your insurance card

The registration process usually takes approximately 30 minutes or less to complete. Appointments are not required for registration.

We encourage you to download, print, and complete both of the following health history forms and bring it with you when you come in to pre-register.
